Semi-synthetic penicillin depends on a single intermediate, 6-APA, produced by fermentation at only seven sites worldwide, five of them in China. Beijing thus controls 80 to 90% of antibiotic APIs and sets the floor price that has driven out Western and Indian fermenters. No backup capacity could absorb a prolonged disruption of Chinese supply.

Nearly all penicillin-class antibiotics, amoxicillin foremost, derive from a single intermediate molecule, 6-APA, produced by fermentation. The West let this fermentation go: the last American plant closed in 2004. Today five Chinese firms concentrate most of it, and even India, the "pharmacy of the world," depends on it for nearly 96 %. In everyday medicine, the breaking point is the fermentation vat, not the pill.

The world believes it depends "only a little" on China, the finished dose comes from there for only ~20 %. But the further up toward fermentation, the more the dependence becomes near-total: the real lock, 6-APA, is ~90 % Chinese. Diversifying downstream does not reduce the risk. The critical point is the fermentation

Making amoxicillin follows a precise chain: glucose and precursors feed a living culture of fungus (Penicillium), which produces penicillin G; this is transformed into 6-APA, 6-aminopenicillanic acid, the common backbone of all semi-synthetic penicillins; 6-APA finally becomes the active ingredient, then the tablet. The decisive link is neither the finished molecule nor the active ingredient, but the fermentation step that produces 6-APA.

This step is biotechnology, not commoditizable chemistry: a living culture to master in bioreactors of 200,000 to 300,000 liters, with heavy process know-how and regulatory validation. You do not reopen a fermentation the way you restart a packaging line. This is where the West lost its footing, and where five Chinese firms, NCPC, CSPC, Lukang, United Laboratories, Harbin, today concentrate more than 80 % of world production.

The "80 % Chinese", what the viral figure really hides

Everywhere one reads that "80 % of US active ingredients come from China." As stated, it is false. The figure traces back to a 2016 GAO report that said something entirely different: about 80 % of the active ingredients consumed in the United States are manufactured outside the country, spread across more than 150 countries, not "in China." In real volume, only around 8 % of US generics contain a Chinese active ingredient.

The true dependence is therefore not uniform: it is targeted and deepens as one moves up the chain. On the finished dose, the diversity is real. On antibiotic active ingredients, China already accounts for around 44 % of global exports. And on 6-APA, one approaches a monopoly. To conflate "registered sites" with "volumes produced," or "all generics" with "antibiotics," is to miss the exact spot where the system is fragile.

India, a "pharmacy of the world" that deceives the eye

India exports drug doses to the whole world and presents itself as the planet's pharmacy. But this autonomy is a one-notch shift: according to India's own institutions, the country depends on China for about 65 % of its pharmaceutical raw materials, 87 % for antibiotics, and up to 96 % for 6-APA. The world's dependence on India is, in large part, a disguised Chinese dependence.

The apparent diversity downstream sustains the illusion. There are more than twenty formulators of piperacillin-tazobactam in China, but a single supplier of the active ingredient for the entire European Union. Benzathine penicillin, the reference treatment for syphilis, has only three active-ingredient makers in the world, all Chinese. Since 2023, no more tazobactam is made in Europe. Multiplying brands diversifies nothing as long as they all draw from the same upstream source.

Why the West left, and does not return

The Western exit was economically rational. The price of antibiotics in finished doses has fallen by about 90 % since the 1990s, that of active ingredients by about 80 %. On a zero-margin generic, no one reinvests in a fermentation. Added to this is a heavy environmental cost, fermentation generates 8 to 10 tons of residue per ton of antibiotic, that the West effectively offshored along with the production.

Attempts to return confirm it. In the United States, the only amoxicillin formulator reopened in 2021 does only the downstream: neither fermentation nor 6-APA. In Europe, the last integrated Western site, Sandoz at Kundl in Austria, survives only on state-aid drip (28.8 million euros notified to Brussels), while simultaneously filing an anti-dumping complaint against Chinese amoxicillin "systematically underpriced." Even subsidized, a Western site cannot hold at market prices: the dependence holds by cost structure, not by accident.
